A Clinical Study of Anal Fistula

Abstract
The clinical study of 138 cases with anal fistula which had been seen at the Dept. of General
Surgery in Capital Army Hospital since Jan. 1965 to July 1970 was made.
The results were as follows:
1. The most common etiological factor of anal fistula was perianal abscess secondary to infectin of
anal crypt (82.6%).
2. Tuberculous origin was confirmed by histopathologiacal examination in addition to chest X-ray,
Sputum examination and was accounted for 4.3% of all cases.
The hemorrhoidal origin of all fistula was accounted for 6.5%.
A good result was obtained in anal fistula with tuberculous origin by fistulotomynd post operative
anti-tuberculous medication.
3. The predilection site of external orifice of anal fistula was posterior part of anus and 32.6% of
all cases was located 1.5¡­2§¯. apart from anal verge.
4. 24.1% among the all cases was multiple, and complete anal fistula was about 3 times to incom
plete type.
5. The main subjective symptoms were discharge, bleeding, pain and discomfortness of anal region
in order.
6. The most common other anal disease associated with anal fistul awas hemorrhoid (41.3%), and
perianal abscess was 7.2% of all cases.
7. The majority of cases were operated by fistulotomy which was easier technically and the post
operative results were better than fistulectomy.
